    • A detailed plan of operation filed with the application

    and competence, good faith and reputation

    • A finding that the applicant is competent and trustworthy and intends to transact its business in good faith, has a good business reputation, and has had experience, training or education qualifying it for the business applied for

    and financial responsibility

    • Evidence of financial responsibility in the form and amount the commissioner prescribes by rule in chapter 284-97 WAC, which the commissioner may call for again at any time

    and standing to do business in Washington

    • For a legal entity, formation or organization under Washington law, or authority as a foreign legal entity to transact business here, or a certificate of good standing from the state of domicile

    and an antifraud plan

    • An antifraud plan meeting RCW 48.102.140, describing how possible fraudulent acts are detected and investigated, how inconsistencies between medical records and insurance applications are resolved, how fraud is reported to the commissioner, and how antifraud staff are arranged and trained

    RCW 48.102.011(1) makes the license territorial in an unusual way: it bites on a person "wherever located" who acts as a provider with an owner resident here, and on a policy with several owners it is enough that one of them lives in Washington. Subsection (7) is the list of findings and subsection (5) the power behind them, letting the commissioner require full disclosure of stockholders, partners, officers and employees and refuse the license over any of them who could materially influence the applicant's conduct. Subsection (6) makes the entity license cover every member, officer and designated employee named in the application. The fee is $250 to apply and $250 to renew, annually on 1 July, with the same 50, 100 and 200 percent surcharges the producer licenses carry. The Office says it issues the license by email within fifteen business days of the last error being resolved.
