Tennessee's provider and broker application is an entity form signed by a president; this one asks for a Social Security number and a date of birth, and it cannot be exercised without a contract with a licensed provider.

    • A viatical settlement investment agent's contract or agreement with a viatical settlement provider, entered into before the applicant engages in the business. The applicant certifies to this on the application itself.

    The Department's own application is personal rather than corporate: it asks for a Social Security number, a date of birth and a home address, and then for the employer separately. The $200 filing fee is nonrefundable. The certification the applicant signs is the whole of the substantive requirement: "I understand that I must enter into a viatical settlement investment agent's contract or agreement with a viatical settlement provider prior to engaging in the business of a viatical settlement investment agent."There is no examination, no course and no bond.
