Certified copies of the original organizational documents and every amendment: articles of incorporation or association, partnership agreements, trade name certificates, trust agreements, shareholder agreements and bylaws, and, for a manager not domiciled here, proof of registration with the Tennessee Secretary of State.

Rule 0780-01-95-.13(1): "No person or entity may act as a PBM in this state without first receiving a PBM license from the Department in accordance with T.C.A. section 56-7-3113 and this rule."The list above is subsection (2), which the rule says is in addition to what the statute asks. There is no examination and no course. The application is filed through the Department's CORE portal, and the license fee is set by Tenn. Code Ann. 56-7-3113(e)(1), which Tennessee does not publish free of charge anywhere this site can read, so no figure is recorded. The fees the rule itself sets are for other things: $1,000 to have an initial appeal process approved, $1,000 to change one, $1,000 for a temporary appeal process, $2,000 for the review of the annual report and $250 to renew a license up to thirty days late. A license expired by more than thirty days cannot be renewed at all and the manager applies again from the start.
