What the written application must show

5 years The proposed seller's name and address and their occupations for the preceding five years; the name and address of the proposed trustee; and the names and addresses of the proposed performers, specifying which services, supplies and equipment each is to furnish under the prepaid contract

A copy of the proposed trust agreement with a written statement signed by an authorized officer of the proposed trustee accepting the trust and confirming it understands its nature; a copy of every contract or understanding between the seller and the performers; a certified copy of the articles of incorporation and bylaws of a corporate applicant; and the non-refundable fee at NRS 680B.010 plus the fees at 680C.110
The proposed seller, or the appropriate corporate officer of it, applies in writing to the Commissioner showing four things: the seller's name and address and their occupations during the preceding five years, the name and address of the proposed trustee, the names and addresses of the proposed performers with the particular services, supplies and equipment each is to furnish, and whatever else the Commissioner reasonably requires. The accompanying documents are the trust agreement with the trustee's own signed acceptance, every contract or understanding between seller and performers, a certified copy of a corporate applicant's articles and bylaws, any other document the Commissioner requires, and the non-refundable fee at NRS 680B.010 together with the 680C.110 fees. A resident natural person files fingerprints at their own expense on the same terms as an agent. The five years of occupations is a disclosure rather than an experience requirement: the statute asks what the applicant has been doing, not that it qualifies them.
