    The accreditation

    • Documentation that the applicant has been approved or accredited by the American Accreditation HealthCare Commission/URAC, or by a similar organization whose standards for utilization review agents are substantially similar and which the director has approved. The qualification is bought from a private accreditor rather than earned from the state.

    and the application

    • A signed and verified application on the Department's form, with the street and mailing address, the telephone number, a list of the principal officers responsible for the entity's operation, management and control, and whatever else the Department reasonably requires.

    and the fee

    • $300 with the application and $100 on each two-year renewal, which is the largest gap between an initial and a renewal fee on the Nebraska schedule.

    Neb. Rev. Stat. 44-5419 requires the certificate and sets its term at two years from issuance; 44-5420 is the application and the fee; 44-5421 is the grant or denial with notice and hearing; 44-5424 is the renewal. There is no examination and no pre-licensing course, and the substantive standards the agent has to meet are the accreditor's rather than the Department's. Georgia's private review agent is the same instrument under a different name.
