The application

A written application on the Department's form naming the employee or authorized representative the vendor designates as responsible for compliance with the Portable Electronics Insurance Act, with that person's residence address, and the location of the vendor's home office. Where the vendor derives more than fifty percent of its revenue from selling portable electronics insurance, the same information is required for everybody of record holding ten percent or more of any class of the vendor's registered securities.

$50 initially and $50 on each renewal, against a statutory ceiling of $100 for each. The license runs a year and expires on 30 April.
Neb. Rev. Stat. 44-8503 and 44-8504, inside the Portable Electronics Insurance Act at 44-8501 to 44-8509. There is no examination and no pre-licensing course. 44-8503(2) is the reach of it: one license authorizes an employee or authorized representative of the vendor at each location where the vendor engages in a portable electronics transaction, so a chain buys one $50 license rather than one per store.
