The application

An application for licensure in the form and manner the director prescribes, with the director empowered to set the licensing, financial and reporting requirements by rule. Neb. Rev. Stat. 44-4605(2) and (3).

The director may refuse to issue or renew where the applicant, or any individual responsible for the conduct of its affairs, is not competent, trustworthy, financially responsible or of good personal and business reputation, has violated the insurance laws of this or any jurisdiction, or has had a certificate of authority or license denied or revoked for cause anywhere.

A nonrefundable application fee the director sets at not more than $500, and an annual renewal fee at not more than $250, due at least thirty days before the anniversary of the effective date. Neither figure is on the Department's published fee schedule.
Neb. Rev. Stat. 44-4605, inside the Pharmacy Benefit Manager Licensure and Regulation Act at 44-4601 et seq. There is no examination and no course. The license has no fixed term: 44-4605(6)(a) keeps it valid "as long as the pharmacy benefit manager continues to do business in this state and remains in compliance", on an annual renewal application and fee. Thirty states here carry a pharmacy benefit manager and Nebraska is one of the few whose fee schedule does not price it.
