The accreditation

Accreditation by a nationally recognized private accrediting entity whose independent review organization standards the director has determined are equivalent to or exceed the minimum qualifications at Neb. Rev. Stat. 44-1313. The director may approve an unaccredited organization only where there is no acceptable accrediting entity to be accredited by.

The Department's application form with all the documentation and information the director needs to decide whether the minimum qualifications at 44-1313 are met, including the independence and conflict of interest provisions that stop a reviewer deciding a case it has an interest in.

$100 for the license and $100 on each renewal on the Department's schedule; 44-1312(4)(c) lets the director charge an application fee.
Neb. Rev. Stat. 44-1312, inside the Health Carrier External Review Act, with 210 NAC 87 as the Director's rule. There is no examination and no course: the qualification is an accreditation and a set of independence findings. It is the same shape as the utilization review agent's certificate two acts away, and Nebraska is one of the few states that licenses both halves of the argument, the agent who denies the care and the organization that reviews the denial.
