    • An application for a certificate of registration on the Department's form, with the fee. Neb. Rev. Stat. 44-8306 sets out the application, the renewal, the disciplinary grounds, the hearing and the penalty in one section.

The act's substance is disclosure rather than qualification: what the card is, what it is not, and that it is not insurance.
