The application

A completed application form submitted online with a $1,000 application fee. The Department states the term in the same breath as the fee: "A Pharmacy Benefits Manager license shall be effective for one year from the date it is issued."

Notice to the Department of any change to the information supplied in the application within sixty calendar days of the change.
Section 376.393, RSMo requires the license, and the Department's Other Licensees page carries the fee, the term and the sixty-day notice duty. There is no examination and no course. Missouri licenses a pharmacy benefits manager as most states here do, and at $1,000 its fee sits in the lower half of the ones read at source: Massachusetts charges $25,000 and Connecticut and Maine $100.
