A detailed statement describing the business plan of operation, with biographical affidavits for every officer, director and key employee of the provider.

A current original certificate of registration or exemption from registration under the Mississippi Securities Act from the Secretary of State. A provider buying policies but not selling to investors may file an affidavit to that effect instead.

A current original certificate of good standing from the Mississippi Secretary of State, a certificate of good standing from the state of domicile for a business entity, and for a nonresident provider a current original letter of certification from the domiciliary insurance department.
Privilege tax $200.00 and renewal $200.00. No examination and no pre-licensing course. The securities condition is the one worth reading twice: the Department will not license the buying of life policies without seeing where the applicant stands with the Secretary of State under the Securities Act, and it accepts an affidavit only from a provider that buys policies and does not sell interests in them to investors. A provider that fails to renew before expiry files a new application rather than a reinstatement. 19 Miss. Admin. Code Part 2, Chapter 15 is the regulation.
