It is a business entity license only: the Department publishes no individual version of it, and it asks for a financial statement affidavit and a Mississippi-specific company information form on top of the national application.

A financial statement affidavit and the Department's Mississippi Specific TPA Company Information form, both published beside the application.
Privilege tax $200.00 for the entity. No examination and no pre-licensing course: neither the Department's list of examinations nor its table of required hours reaches this license. Mississippi issues it only to a business entity, where it issues both an individual and an entity version of the producer, the limited lines producer, the credit producer, the managing general agent, the supervising general agent, the reinsurance intermediary, the viatical representative and both adjusters.
