The Department publishes instructions and two application forms for it, one for an individual and one for an entity, under Additional Licensing Types. It is a permit rather than a license in the Department's own wording, and its fee is not on the licensing fee schedule, which prices licenses and entity licenses and stops. No examination and no pre-licensing course: neither the examination list nor the hours table reaches it. Recorded here because the Insurance Department issues it and the subject is insurance on a car somebody else is driving, which is the same scope test the transportation network company license passes.
