    The sworn application

    • A sworn application on the superintendent's form naming the employee or authorized representative responsible for the provider's compliance with the chapter, giving the location of the home office, and appointing the superintendent irrevocably as attorney to receive service of legal process; where more than half the provider's revenue comes from selling this insurance, the same information for every officer, director and holder of ten percent or more of any class of securities registered under federal securities law

    and the list of locations

    • A list of every location in Maine at which the provider offers coverage, including any publicly accessible website or call center directed at Maine residents

    24-A M.R.S. 7602(1) requires the license before any cover is sold and 7602(2) then reaches every employee and authorized representative at every location the provider deals with customers from, which is the portable electronics shape one chapter later. 7606 is the application and three of its subsections are unlike anything else in Title 24-A. 7606(2)(B) makes the appointment of the superintendent as attorney for service of process irrevocable and binding on any successor in interest and on the applicant's assets and liabilities, for as long as the license is in force. 7606(3) puts a 90-day deadline on the applicant rather than on the Bureau: an application "must be made within 90 days of the application's being made available by the superintendent". And 7606(4) fixes the initial term at 24 months expiring on the last day of the 24th month, so this license runs on its own clock rather than on a common renewal date. The chapter number is worth knowing before searching: Maine's limited lines self-storage insurance sat at chapter 97, sections 7501 to 7506, that chapter was repealed, and the live law is chapter 99 at sections 7601 to 7606. The Title 24-A contents page still lists the repealed chapter 97 immediately above a second, unrelated chapter 97. The fee is the smallest of Maine's limited lines entity licenses, $100 to issue and $50 a year at 601(33), against $1,000 and $500 for the portable electronics vendor.
