Viatical Settlement Provider
Buys a life policy from the person insured under it, or finances somebody else who does. It is the only one of Indiana's three viatical roles that is actually licensed, and it costs a thousand dollars and asks for a hundred and fifty thousand of net worth.

-
Organization documents including articles of incorporation or association, the partnership agreement and the by-laws, originally signed and notarized biographical affidavits for every officer, director, stockholder, investor, partner, trustee or member holding ten percent or more of the voting securities, a list of all business licenses, and a listing of parent, subsidiary and affiliated entities.
and the forms, approved before use
-
A copy of every viatical settlement contract to be used with the Department's contract compliance checklist, a copy of every disclosure form with its own checklist, and a plan of operation covering escrow accounts and banks, advertising and marketing techniques. A nonresident applicant also files a uniform consent to service of process.
IC 27-8-19.8-1 and 760 IAC 1-61-1. The application and every document go by postal mail, which the Department states outright, and it is the only Indiana license class here that cannot be filed electronically. The other two viatical roles are not licensed and the Department answers that itself in its own FAQ: a viatical settlement agent or broker files a declaration and "The declaration is not a license, rather it is a document that declares the individual or company will be acting as a agent/broker in the state. As long as the individual has a current life license, the broker/agent viatical declaration will remain active."It is perpetual, it costs nothing, and it rests on an Indiana life producer license the person already holds, which is Iowa's test answered by the department in one sentence.
