An electronic application at Sircon's Indiana site with the broker checklist, the manager checklist, a uniform consent to service of process and a biographical affidavit attached. The last two of those four are the ones the Department marks as required for managers only.
IC 27-6-9 and 760 IAC 1-51. No examination and no course. Indiana joins Michigan, Alaska, Texas, Virginia, Hawaii, Pennsylvania and Massachusetts on two reinsurance intermediary licenses rather than one, and it is unusual in issuing both off a single page and a single electronic form with the manager's extra requirements marked by an asterisk.
