The bond

A surety bond equal to at least 10 percent of the annualized funds handled or managed, and never less than $100,000, in favor of the Commissioner, with 30 days' notice of cancellation to the Office. Where the applicant both collects premiums and pays claims the 10 percent is taken on the greater of the two.

A Biographical Statement and Affidavit and an Authorization for Release of Information for every individual holding more than 10 percent and everyone responsible for the conduct of the administrator's affairs, with an investigative background report from the approved NAIC vendor for each. Audited financials for the past two calendar years, or a statement of the amounts and sources of the funds establishing the business where the applicant is not yet trading.
O.C.G.A. 33-23-100(a)(1) defines the administrator by what it does with somebody else's money: collecting charges, fees or premiums; adjusting or settling claims, including investigating them or handling claim funds; or providing underwriting or precertification and preauthorization of hospitalizations or medical treatments, for or on behalf of an insurer. Licensure reaches an administrator acting for a single or multiple employer self-insurance health plan, for a self-insured municipality or political subdivision, and for a self-insured workers' compensation plan under Chapter 9 of Title 34. Each activity on behalf of a client is a separate transaction under the title. $500 to apply and a $5 processing fee; $400 and $5 to renew, and a rules change in July 2025 moved the annual renewal to 1 August.

O.C.G.A. 33-23-100(b) exempts thirteen kinds of entity so long as they act directly through their own officers and employees: an employer for its own or an affiliate's employees; a union for its members; a Georgia-licensed insurer or its affiliate, unless the affiliate places business with an unlicensed nonaffiliate; an unauthorized insurer administering a policy it lawfully issued elsewhere; a licensed life or accident and sickness agent or broker doing nothing but selling; a creditor for its debtors on credit-related cover; a Taft-Hartley trust under 29 U.S.C. 186; a tax-exempt trust under section 501(a) or a custodian under section 401(f); a bank, credit union or other supervised financial institution; a credit card issuer that advances and collects premiums but settles no claims; a person adjusting claims in the normal course of practice as an attorney who collects no life or accident and sickness premiums; an insurer or affiliate administering only limited benefit insurance; and an association administering workers' compensation claims solely for its members.

The Office's exemption form signed by an officer of the entity, filed through Sircon with a $50 registration fee and a $5 processing fee, and renewed by 31 December every year at the same $50 and $5. The renewal asks the entity to copy its exemption reason out in full and paste it into the answer box.
This is the shape worth carrying: an exemption from licensure that is itself an application, priced, dated and renewable. Maryland prices its ERISA-only administrator's row $0 and takes a notice; Georgia charges $55 for the same idea and calls the result a Third Party Admin - Exempt license in its own renewal menu.
