A copy of the proposed premium finance agreement form, which must comply with O.C.G.A. 33-22-8 and 33-22-12, filed with the application.

The charter, articles of incorporation and bylaws, a certificate of good standing or existence from the Georgia Secretary of State, the Annual Report of Insurance Premium Finance Companies with certified financial statements or a balance sheet, a list of the Georgia agents and their license numbers for each officer or manager, and an investigative background report for each individual, covering ten years of residences and any local, state or federal court findings.
The Office describes the licensee as an agent or agency that enters into an insurance premium finance agreement under which an insured or prospective insured promises to repay the amount advanced to an insurer, agent or broker in payment of premiums, together with a service charge. A company financing only the premiums its own agency produces says so in a letter naming both. $500 and a $5 processing fee for Full Powers, $300 and $5 for Limited Powers. Licensing and renewals moved to Sircon on 2024-11-15. There is no examination and no course.
