A certificate of authority from the fraternal benefit society, required at the time of application rather than after licensure, with the application submitted by the sponsoring company. The license covers life and accident and sickness.
The Office's license table gives this row an empty education column, no exam code and the note "Submitted by Sponsoring CO". Every other agent license in the table reads "Company CA required after licensure"; this one reads "Company CA required at time of application", which reverses the order the rest of the chapter uses. The Office's continuing education chart then exempts fraternal agents from continuing education entirely, which it does for eight other things on the same list: temporary agents, foreign military representatives, title agents, the four nonresident designated home state adjuster licenses and non-active agents.

No national seller sells a course for this license. There is no course and no paper to prepare for: the society appoints the agent and files the paperwork.
