    • Organizational documents and bylaws; the federal identification number and addresses; names, positions, biographies and conflict disclosures for everybody responsible for the applicant’s affairs and every 10 percent owner; a description of the applicant and the services to be discounted; the forms of every provider contract; and a surety bond or deposit

    18 Del. C. 7603(a) and (b). Nothing in the section is an examination or a course, and nothing in it is asked of an individual: the licensee is the organization. What it does ask for is the paperwork of a company rather than the qualification of a person, down to the form of every contract with a provider or provider network, and a disclosure of the extent and nature of any contract between the organization and each individual who runs it. A surety bond or deposit follows at 7604.
