The license

A discount medical plan organization license under SDCL chapter 58-17E. The Division publishes a license application requirement checklist and an additional information page.
and the surety bond

A DMPO surety bond on the Division's own form, which it publishes beside the checklist. It is the only one of the six instruments on South Dakota's Other License Types page that carries a published bond form.

58-17E-8 requires notification of the director where a discount medical plan organization loses something the chapter requires it to keep.
