    S.C. Code 38-71-2210(A)(1) forbids establishing or operating as a pharmacy benefits manager for health benefit plans without the Director's license, and (A)(2) leaves the form to the Director while fixing what it must collect: the manager's name, address and telephone number, its agent for service of process here, and the name and address of every person with management or control over it. The fee is a ceiling rather than a price, the Director MAY charge up to $1,000 initially and $500 a year after. There is no examination and no course anywhere in the article.
