    • A public insurance adjuster agent sponsorship form filed by the licensed public insurance adjuster the applicant is a bona fide employee of.

    and bond

    • Proof of a bond, filed on the Department's own form with the application and again with every renewal.

    The Department defines the agent as "any person who is a bona fide employee of a public insurance adjuster and who aids in the adjustment, investigation, and securing of any contract for the adjustment of a loss", so this license cannot exist on its own: it is held by somebody working for somebody else who holds the other one. It takes the same Public Insurance Adjuster examination, which is one of the nine papers the Department offers, and needs no pre-licensing course: division (C) of Revised Code 3905.04 does not reach it. Both licenses expire on 31 December every year, and the fee is $50 against the adjuster's $100. Since 30 September 2025 neither application has to be notarized.
