    Contract with the insurer

    • A copy of the contract between the applicant and the insurer, complying with Ohio Revised Code 3905.73.

    and board resolution

    • A certified resolution of the insurer's board of directors appointing the applicant as managing general agent and agent of the insurer, specifying the duties expected and the lines the applicant will manage, and authorizing the insurer to enter the contract.

    • 5 years For an individual applicant, the applicant's current occupation and every occupation held in the five years before applying.

    Ohio Revised Code 3905.72(B) is the application and 3905.71 is the definition, which turns on volume rather than on title: a managing general agent produces and underwrites gross direct written premium equal to five per cent or more of the insurer's policyholder surplus in a year, and manages part of the insurer's business, adjusts or pays claims, or negotiates reinsurance. Ohio issues the license to an individual or to a business entity, and the license expires annually on the last day of February. No examination and no course. Since 30 September 2025 the application no longer has to be notarized. The occupational history is the applicant's own and the five years is the statute's; nothing here is an experience requirement in the sense of qualifying, it is a disclosure.

Ohio sets no examination and no course for the managing general agent. No national seller prices a product for it.
