A statement describing the organization, its enrollment process, its administrative operations, its cost and quality control assurance mechanisms, its internal grievance procedure, how it proposes to enroll members, the geographic areas served, its offices, the number of providers to be used and the recordkeeping system documenting members' use of benefits.

A statement of the organization's financial condition, with income statement, balance sheet and sources of funds, and a copy of its complaint system.
N.D.C.C. 26.1-19-04(1): a person may not establish or operate a prepaid legal services organization in this state, or sell, offer to sell or solicit offers for a prepaid legal services plan, without a certificate of authority. The commissioner issues it under 26.1-19-06. There is no examination and no course. N.D. Admin. Code 45-13-01-04 says the same thing from the other end, that an insurance company marketing prepaid legal services must hold this certificate before transacting business here even though prepaid legal service is a product inside the Casualty line at 45-13-01-02(4). The Department prices the application at $500 and asks each year for an audited financial statement, any changes since the application, and a count of complaints by type with their disposition.
