    • The principal office and the agent for service of process; the executive officers directly responsible for the service contract business, and, where more than half the provider's revenue comes from selling service contracts, its directors and any holder of five percent or more of a registered class of securities; the name, location and telephone number of every administrator, with each unemployed administrator's acknowledgement of its own obligations; and a statement of which financial-responsibility ground under Insurance Law 7903(c) the provider qualifies on.

Insurance Law 7907 sets no examination and no course. The superintendent has forty-five days to decide, and the provider has thirty days after the end of any month in which it learns of a material change to bring the filing up to date. New York calls this an approval of a registration rather than a license, and 7907(a) forbids acting as a provider until that approval is in hand, which is why it sits here beside the licenses rather than outside them.

New York sets no examination and no course for a service contract provider. No national seller prices a product for it.
