    • The state of domicile, the principal place of business, every other state the intermediary does or intends to do business in, a detailed plan of operation, and the identities of the executive officers directly responsible for the business along with every stockholder holding ten percent or more of the voting shares, partner, officer, member, director and person with a controlling interest.

    Insurance Law 7804 sets no examination and no course. It is a registration rather than a license and the Department words it that way throughout, which is the same distinction it draws for a service contract provider. The registration expires on 30 June of odd-numbered years. Filing goes by email with a paper check, and a power of attorney designating the superintendent as agent for service of process is part of the application. Biographical affidavits go by mail in original form, and electronic fingerprinting evidence is filed with the rest.
