The name, address and telephone number of each administrator the provider intends to contract with, if any.
NMSA 59A-58-4 forbids the sale of a service contract unless the provider is registered, and 59A-58-5 is the registration. The fee sentence is unusual enough to quote: the statute asks for "provided that House Bill 248 of the first session of the fifty-fifth legislature: (a) becomes law, the registration renewal fee provided in Section 59A-6-1 NMSA 1978; or (b) does not become law, a fee of five hundred dollars". The Insurance Code's fee schedule prices no service contract provider, so the second limb is what applies and the figure is $500. A registration is valid for one year from the date it is filed. There is no examination and no course.
