The identity of the pharmacy benefits manager, the name and business address of its contact person, its federal employer identification number where applicable, and anything else the superintendent requires by rule.
NMSA 59A-61-3: a person may not operate as a pharmacy benefits manager unless licensed by the superintendent, and the license is renewed annually rather than biennially. The statute sets a floor for the application rather than a ceiling, listing four things it "shall at a minimum require" and leaving the rest to rules. The fee schedule at 59A-6-1(AA) is $1,000 to apply, $500 for each year continued, $200 for each annual report and $200 for an amendment to organizational documents needing approval. New Mexico also regulates the other side of that market: 59A-61-8 requires a pharmacy services administrative organization to register, which is a separate instrument from this one. There is no examination and no course.
