The name, residence address and other details of one employee or officer designated as responsible for the vendor's compliance, or, where the vendor draws more than 50 percent of its revenue from selling this insurance, the same for every officer, director and holder of ten percent or more of any registered class of its securities

A statement of whether the vendor intends to offer portable electronics insurance from more than ten Minnesota locations, which sets the fee at $100 per location up to ten and $6,500 above it; a vendor offering cover through a website is deemed to be selling from more than ten

A registry of every location authorized to sell or solicit the cover in Minnesota, kept by the vendor or by the supervising entity appointed to oversee the program, produced for the commissioner within ten days of a request
Minn. Stat. 60K.381, subd. 2 licenses the vendor and subdivision 4 then holds the counter staff out of licensure entirely, on three conditions: the vendor holds the limited lines license, the insurer either supervises the program directly or appoints a supervising entity that builds a training program for every employee and authorized representative who sells the cover, and nobody holds themselves out as a non-limited-lines producer. The training is the insurer's or the supervising entity's, not a school's, and it carries no hour count. Where it is delivered electronically the supervising entity has to run a supplemental education program conducted by its own licensed employees, which is the only place in Minnesota's producer chapter where the delivery of training changes what is required. The fee at 60K.55, subd. 2 is the widest spread on the Minnesota schedule: $100 per location up to ten, and $6,500 flat above ten.
