The Business Entities Application on Form PEI, and where the retailer takes more than fifty per cent of its revenue from selling portable electronics insurance, contact information, ownership percentage and title for every officer, director or shareholder holding ten per cent or more of it, on Form PEI-Locations
806 KAR 9:390 Section 2 is the whole application: the Business Entities Application on Form PEI, the ownership disclosure on Form PEI-Locations where more than half the retailer's revenue comes from selling this insurance, and the fees at 806 KAR 4:010. There is no course, no hour count and no examination. Section 1 takes its definitions straight from KRS 304.9-020(16) to (19), which is where portable electronics, portable electronics insurance, the supervising entity and the retailer are each defined. The duty to keep a register of locations at Section 3 falls on the supervising entity rather than on the retailer, which is the same shape as the training duty in a rental car limited license: a duty on somebody else in the chain. 806 KAR 4:010 section 1(8)(b) prices the license by size, at $100 per location for one to twenty Kentucky locations and $2,500 in total for twenty-one or more, so the twenty-fifth location is where paying per location stops being cheaper.
