    K.S.A. 40-3823(b) sets out the minimum contents of the application and none of them is a qualification of a person: the applicant's details, the name and professional qualifications of everybody responsible for the conduct of its affairs, an agent for service of process, a named employee to be the Department's primary contact, the corporate charter, a template contract carrying a dispute resolution process that ends with an independent fact finder, and a network adequacy report on the form K.A.R. 40-1-54 prescribes. The fee is nonrefundable and capped at $2,500, which is the largest application fee in the Kansas Insurance Code. The Commissioner has 90 days from a complete application to review it, and a licensee that fails to report a material change within 90 days can be penalized $500. K.S.A. 40-3821(d) carves out anybody holding a certificate of registration as an administrator under 40-3810.
