The registration

A non-refundable fee not exceeding $500, a copy of the corporate charter, articles of incorporation or other charter document, and a registration form giving the name and address of the registrant and the name, address and official position of each of its officers and directors. The certificate runs two years and is nontransferable, and any change in the information goes to the Director in writing within 60 days.

An annual report naming each health benefit plan the manager administers and the number of Illinois residents covered under each, and a payment of $15 for every such covered individual, or an alternate amount the Director sets by rule. It goes into the Prescription Drug Affordability Fund, whose first $25,000,000 each year is transferred out to fund grants supporting pharmacies.
215 ILCS 5/513b2(a) has required registration since 2020-07-01 and (d) caps the initial and the renewal fee at $500 apiece, with total fees not exceeding the cost of administering the section. Subsection (f) is the newer half and it is much the larger number for anybody of size. 513b5 makes operating without registering an offence in its own right. There is no examination and no course, and Illinois registers rather than licenses: the Article's own heading for the enforcement section is "Denial, revocation, or suspension of registration".
