A notice published in at least one newspaper with substantial circulation in the service area, naming the medical specialties the network covers, where providers apply to join and by when, under Conn. Gen. Stat. 38a-479aa(c)
Conn. Gen. Stat. 38a-479aa(b) forbids a preferred provider network to enter into or renew a contract with a managed care organization or conduct business here unless licensed, and fixes the calendar: the license issues or renews on 1 July and the application has to be in by 1 May to make that date. The filing carries a certificate of good standing for the network and for whatever company or organization controls it, financial statements prepared under 38a-53 and 38a-54 with the accountant named, and the names, positions and occupations of the board and of the executive officers responsible for the health care services network. Four things are defined out of the license rather than into it, at 38a-479aa(a)(7): a workers' compensation preferred provider organization set up under the labor regulations, an independent practice association or physician hospital organization whose main job is contracting with insurers, a licensed clinical laboratory paying mostly other laboratories, and a pharmacy benefits manager. No course and no examination.
