The Department publishes five forms for this license and instructions covering all of them: the application on AID-LI-MGA 40 for a corporation, limited liability company, limited liability partnership or partnership, the company appointment on 41, the termination of that appointment on 42, the bond on 43 and the contract itself on 45. Rule 57 Section 18 prices it at $500 for the initial license and $500 again at every annual renewal, both Category A fees, and Rule 57 Section 5(a)(5) charges the insurer a separate $500 to file the notice of appointment or of termination. There is no course and no examination: the vendor gives no managing general agent paper and Rule 31 has no syllabus for one.
